Key Responsibilities

• Receive and review physician therapy orders for Home Health and Hospice patients
• Complete comprehensive patient evaluations and establish written treatment plans with short-term and long-term goals
• Provide skilled physical therapy interventions, including therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation
• Assess home safety and recommend equipment, assistive devices, or environmental modifications to promote independence and prevent falls
• Educate patients, caregivers, and family members regarding treatment plans, home exercise programs, safety measures, and expected outcomes
• Maintain accurate and timely clinical documentation, including evaluations, progress notes, discharge summaries, recertifications, resumptions of care, and discharge documentation

Required Qualifications

• Current Physical Therapist licensure in the state of Montana
• Valid driver's license required for travel to patient homes and other care settings
• BLS Certification required

Preferred Experience / Skills

• Prior experience in a home health or hospice therapy setting
• Experience collaborating with multidisciplinary care teams including physicians, nurses, and other therapists
