    Contract | Days (5x8 Hours, 8:00–17:00) | Onsite / Field-Based

    Key Responsibilities
    • Conduct skilled home health visits, completing a minimum of 6–8 patient visits per day across a variety of diagnoses including orthopedic, neurological, cardiac, and post-surgical conditions
    • Perform comprehensive patient assessments including OASIS documentation and develop individualized home exercise programs
    • Provide patient and family education on safety, injury prevention, signs and symptoms of abuse, and medication management
    • Utilize and assess adaptive equipment and devices including gait belts, walkers, wheelchairs, sliding boards, and upper extremity prosthetics
    • Complete accurate and timely clinical documentation using facility-designated systems and portable technology provided in the field
    • Participate in weekend coverage on a rotating basis (approximately 1–2 weekends per contract)
    Required Qualifications
    • Active Florida PT license or valid compact state license required at time of submission
    • Minimum of 1 year of physical therapy experience, with home health experience required
    • Current BLS certification
    • Valid driver's license and reliable personal vehicle for conducting home visits
    • COVID-19 vaccination or approved exemption on file
    • Flu shot or signed declination (mandatory mask wearing required if declined)
    Preferred Experience / Skills
    • OASIS training and experience strongly preferred
    • Experience with Medicare, Medicare Advantage, and commercial insurance skilled home care
    • Familiarity with Cerner charting systems
