Key Responsibilities
- Provide physical therapy services to patients across a multi-county home health territory
- Meet a minimum productivity expectation of 30 points per week
- Complete all clinical documentation by 10:00 AM the following day
- Utilize HCHB (Homecare Homebase) for all documentation and patient management
- Deliver patient-centered care across assigned geographic service areas
Required Qualifications
- Active Physical Therapist (PT) license
- At least one year of recent home health experience
- Proficiency with HCHB (Homecare Homebase) is required
- Ability to travel across a large multi-county service territory
Preferred Experience / Skills
- Experience managing a high-volume caseload in a home health setting
- Strong time management skills with the ability to meet documentation deadlines
- Familiarity with rural and suburban patient populations across diverse geographic areas
