Key Responsibilities

• Provide physical therapy services across a multi-county rural territory, primarily serving elderly patients with conditions such as falls and fractures
• Complete approximately 4–6 patient visits per day based on a point-equivalent system
• Conduct OASIS Assessments and manage Start of Care cases
• Document time and visits using a provided tablet

Required Qualifications

• Active Physical Therapist (PT) license — must be in hand at time of submission
• Home health experience required
• Home Care Home Base (HCHB) experience required
• Ability to perform OASIS Assessments and Start of Care evaluations

Preferred Experience / Skills

• Experience treating geriatric patients, including fall prevention and post-fracture rehabilitation
• Comfort working independently in a rural, multi-county service area
