Key Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions in a home health setting
- Complete evaluations to ensure quality standards and address assessment areas requiring attention
- Establish individualized treatment plans using appropriate clinical techniques, tools, and protocols
- Develop individualized care and discharge plans and communicate them to appropriate parties
- Complete comprehensive and accurate clinical documentation within established deadlines
- Supervise assigned therapy assistants and serve as co-leader in an interdisciplinary approach to patient care
Required Qualifications
- Active Hawaii state Physical Therapist license
- Minimum of 2 years of Physical Therapist experience
- Minimum of 1 year of home health Physical Therapy experience
- Bachelor's degree in Physical Therapy or related field
- Experience with Epic EMR
- Current CPR and BLS (AHA) certification
Preferred Experience / Skills
- Master's degree in Physical Therapy or related field
- Prior travel therapy assignment experience
