    Contract | Full-Time | 40 hours per week, Monday–Friday, Day Shift (8:00 AM – 5:00 PM) | Onsite | Minimum 12 weeks

    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction.
    • Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
    • Design functional treatment programs that support effective patient return to community activities, ensuring care is provided under valid physician orders and within insurance approvals.
    • Delegate and direct all discipline-specific therapy services for assigned patients.
    • Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations.
    • Actively pursue professional development through continuing education, self-study, and involvement in professional activities and organizations.
    Required Qualifications
    • Active and unrestricted Physical Therapist licensure in good standing in the applicable state.
    • Adherence to discipline-specific State Practice Act and all relevant regulatory agency requirements.
    • Completion of all state-mandated continuing education requirements.
    • Demonstrated clinical reasoning skills to develop appropriate treatment plans and goals focused on progressing patients toward discharge.
    • Commitment to patient and employee safety standards, including safe patient handling and infection control practices.
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting.
    • Experience supervising or mentoring physical therapy students.
    • Familiarity with performance improvement methodologies and quality improvement initiatives.
