Contract | Full-Time | Days | 5x8-hour shifts | Expected 40 hours/week (32 hours during holiday weeks)
Key Responsibilities
- Provide direct physical therapy services to patients within an outpatient setting
- Complete required documentation in accordance with facility and credentialing standards prior to and throughout the assignment
- Deliver evidence-based therapeutic interventions aligned with patient goals and plan of care
- Maintain compliance with all credentialing, licensure, and facility-specific requirements
Required Qualifications
- Active Physical Therapist license — must obtain a permanent Oregon state license promptly; compact licensure may be accepted at start
- Prior experience in an outpatient physical therapy setting
- Ability to meet facility credentialing and contract QA requirements before assignment start
- Candidates who have worked at this facility (permanent or PRN) within the past year are not eligible for this assignment
Preferred Experience / Skills
- Previous travel therapy experience
- Familiarity with outpatient orthopedic or sports rehabilitation settings
