Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Deliver hands-on therapeutic interventions to residents in a skilled nursing setting
• Monitor and document patient progress, adjusting treatment plans as appropriate
• Collaborate with interdisciplinary care team to support patient goals and outcomes
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Graduate of an accredited Physical Therapy program
• Ability to work a Monday-Friday schedule, approximately 35 hours per week

Preferred Experience / Skills

• Prior experience in a skilled nursing or post-acute rehabilitation setting
• Familiarity with geriatric patient populations
• Experience with electronic medical records (EMR) documentation
