    Key Responsibilities
    • Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs, developing appropriate plans of care in accordance with regulatory and clinical practice standards
    • Utilize standardized assessments and evidence-based practice to guide clinical interventions including manual techniques, therapeutic exercises, ambulation, modalities, assistive devices, and wound care
    • Assess and document patient progress, adjust treatment plans accordingly, and evaluate outcomes for effectiveness in the electronic documentation system
    • Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders
    • Participate in required meetings such as patient care conferences, utilization review, and rehabilitation meetings, and complete comprehensive discharge summaries per regulatory requirements
    • Supervise and support physical therapist assistants, rehabilitation aides, and/or students as needed
    Required Qualifications
    • Active Massachusetts Physical Therapist state license required
    • Minimum 1 year of physical therapy experience required; new graduates with completed Level II clinical experience (not solely Level I observation) will be considered
    • Long-term care or rehabilitation setting experience required
    • Adherence to APTA and state-required practice acts, code of conduct, and ethics
    Preferred Experience / Skills
    • Experience with electronic documentation systems in a clinical setting
    • Familiarity with DME procurement and regulatory compliance in a long-term care environment
    • Experience supervising physical therapist assistants or rehabilitation aides
