    Key Responsibilities
    • Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs, developing appropriate plans of care in accordance with regulatory and clinical practice standards.
    • Administer rehabilitation procedures including manual techniques, therapeutic exercises, ambulation, modalities, use of supportive and assistive devices, and wound care.
    • Assess and document patient progress, adjust treatment plans accordingly, and evaluate treatment outcomes for effectiveness in the electronic documentation system.
    • Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders and discharge planning.
    • Communicate effectively with supervisors and the broader health care team regarding patient progress, barriers, and treatment plans.
    • Supervise and support physical therapist assistants, rehabilitation aides, and/or students as needed, and participate in required meetings, trainings, and in-services.
    Required Qualifications
    • Active Massachusetts Physical Therapist state license required.
    • Minimum of 1 year of physical therapy experience, or new graduate with completed Level II clinical experience.
    • Experience in long-term care (LTC) or rehabilitation settings required.
    • Adherence to APTA standards, state practice acts, code of conduct, and professional ethics.
    Preferred Experience / Skills
    • Familiarity with standardized assessments and evidence-based clinical interventions.
    • Experience with electronic documentation systems and accurate recording of treatment minutes per regulatory requirements.
