Contract | Full-Time | Days | 5x8-hour shifts | Expected 40 hours/week
Key Responsibilities
- Evaluate and treat patients in an outpatient physical therapy setting
- Develop and implement individualized treatment plans based on patient needs and clinical findings
- Complete required clinical documentation accurately and in a timely manner
- Collaborate with interdisciplinary team members to support patient progress and care goals
- Maintain compliance with facility credentialing and documentation requirements prior to and throughout the assignment
Required Qualifications
- Active Physical Therapist license — a permanent Oregon state license is required; compact licensure may be accepted at start but permanent Oregon licensure must be obtained promptly
- Prior experience in an outpatient physical therapy setting
- Ability to meet facility-specific credentialing requirements before assignment start
Preferred Experience / Skills
- Experience working in a high-volume outpatient or clinic-based environment
- Strong documentation skills and familiarity with electronic medical records
