Key Responsibilities
- Provide physical therapy evaluation and treatment services to residents within a skilled nursing facility setting
- Develop and implement individualized treatment plans in accordance with physician orders and facility protocols
- Document patient progress, treatment notes, and discharge planning in compliance with regulatory standards
- Collaborate with interdisciplinary care team to support resident rehabilitation goals
Required Qualifications
- Active New York State Physical Therapist license required
- BLS certification required
- Prior experience working in a Skilled Nursing Facility required
Preferred Experience / Skills
- Experience with geriatric patient populations
- Familiarity with long-term care documentation and compliance requirements
