Key Responsibilities

• Provide physical therapy evaluation and treatment services to residents in a skilled nursing facility setting
• Deliver post-hospital, short-term, long-term, and respite care rehabilitation services
• Develop and implement individualized treatment plans to maximize patient functional outcomes
• Collaborate with interdisciplinary care teams to support resident goals and transitions of care
• Participate in weekend rotation coverage as scheduled

Required Qualifications

• Active Physical Therapist (PT) license in the state of Virginia
• Graduate of an accredited Physical Therapy program
• Ability to work a full-time schedule of 40 hours per week

Preferred Experience / Skills

• Prior experience in a skilled nursing facility or post-acute care environment
• Experience treating a diverse patient population including short-term rehab and long-term care residents
