    Job Title: Home Health Physical Therapist Profession: Physical Therapist Specialty: Home Health Duration: Full-time Shift: Monday-Friday Hours per Shift: 8:00 AM to 4:30 PM Experience: One year experience caring for a similar patient population is required License: Licensed as a Physical Therapist in the state where practicing Certifications: Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided Must-Have: Graduate of a Physical Therapist education program approved by the Commission on Accreditation in Physical Therapy Education or an equivalent organization. Ability to perform physical therapy assessment and treatment in the home setting without direct supervision. Strong interpersonal skills. Ability to work as a collaborative team member. Ability to understand and apply guidelines, policies, and procedures. Ability to navigate and perform basic use of Microsoft Office products. Maintains a reliable mode of transportation and has the ability to drive safely during all day and night hours and in all types of inclement weather. Description: The Physical Therapist (PT) uses clinical expertise, physical assessment, teaching, and skilled therapy interventions to deliver high-quality patient care in the patient's place of residence. The PT develops or contributes to the development of the home health plan of care under the direction of the physician. The PT facilitates appropriate interdisciplinary involvement to ensure the best outcome for each patient. The PT delivers therapy intervention and treatment directed by the physician as established in the home health plan of care consistent with clinical best practices. The PT assesses the patient's level of functioning by applying appropriate tests. The PT treats clients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures. The PT ensures quality and safe delivery of services to patients according to the care plan. The PT promotes patient independence by teaching patients and family members about treatment, disease management, medications, proper equipment use, and emergency preparedness. The PT provides counseling and education to patients, caregivers, and family members as indicated in the plan of care. The PT develops and continually updates a plan of care specifying care and services required to meet patient-specific needs determined during assessment. The PT maintains accurate and timely documentation of clinical records and time entries. The PT communicates with all physicians and health care practitioners involved in the care plan. The PT identifies patient risks for hospitalization and proactively intervenes to prevent adverse events. The PT supervises home health aide and physical therapy assistant staff and documents supervision in the patient medical record. The PT delivers quality care and completes clear, concise, and accurate clinical documentation in accordance with home health conditions of participation. The PT participates in the clinical review of records to monitor quality and accuracy.
