The Physical Therapist develops or contributes to the development of the home health plan of care under the direction of the physician and facilitates interdisciplinary involvement to ensure optimal patient outcomes. The Physical Therapist delivers therapy interventions that align with clinical best practices, resulting in improved patient experiences. Key responsibilities include making visits to patients’ homes for ongoing interdisciplinary assessments. The Physical Therapist performs initial and comprehensive assessments on all patients per agency policies and procedures. Evaluating the patient's level of functioning and applying appropriate tests is essential. The Physical Therapist treats clients to relieve pain and restore function, utilizing physical therapy treatments outlined in the care plan. Diagnostic, preventative, and rehabilitative procedures are initiated based on the patient's care needs. Quality and safe service delivery according to the care plan are paramount. The Physical Therapist promotes patient independence by educating patients and family members on treatment, disease management, medications, equipment safety, and emergency preparedness. The Physical Therapist provides services as ordered by the physician according to the care plan. Patient, caregiver, and family counseling and education are integral components of the care plan. The Physical Therapist continuously updates a plan of care that addresses specific patient needs and measurable outcomes. Discharge planning is updated at every visit, ensuring that patient and caregiver education supports timely discharge. The Physical Therapist maintains accurate and timely documentation of clinical records and communication with physicians involved in the care plan. Identifying risks for hospitalization and proactively intervening to prevent adverse events is critical. Supervision of home health aides and physical therapy assistants is also part of the role. The Physical Therapist is responsible for delivering quality care and completing clear, concise, and accurate clinical documentation in accordance with applicable standards. Competence in data collection and documentation is required. Participation in clinical reviews to monitor quality, accuracy, and care coordination is essential.
