Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Conduct 60-minute evaluations with 30-minute follow-up appointments; progress patients toward discharge within an established length of stay
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients; ensure care is provided under valid physician orders and within insurance approvals
• Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, including supervision, development of learning objectives, and completion of student evaluations
• Adhere to the applicable State Practice Act and all regulatory requirements; maintain professional licensure in good standing and fulfill all continuing education requirements

Required Qualifications

• Active Physical Therapist (PT) license in the state of Colorado, in good standing
• Graduate of an accredited Physical Therapy program
• Ability to work across two outpatient clinic locations
• Strong clinical reasoning skills with the ability to develop and progress individualized treatment plans
• Proficiency in medical record documentation and compliance with state, federal, and payer guidelines

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring PT students
• Demonstrated commitment to professional development through continuing education and involvement in professional organizations
