Key Responsibilities
Provide physical therapy services to patients in a home health setting across an assigned multi-county coverage area
Manage a caseload of approximately 5–6 units per day, including start of care, discharge, recertification, and regular visits
Treat patients with diagnoses including general orthopedic conditions (hips, knees), generalized weakness, strengthening needs, and cardiac rehab
Complete clinical documentation accurately and in a timely manner using the Epic system
Coordinate scheduling and patient visits according to geographic area while adhering to initial and reassessment guidelines
Required Qualifications
Active Physical Therapist licensure meeting all requirements for practice in the state of West Virginia
Current Basic Life Support (BLS) certification
Experience performing standard physical therapy evaluations and treatments in a clinical or home health setting
Ability to travel across an assigned multi-county service area
Preferred Experience / Skills
Prior home health experience
Familiarity with or willingness to learn Epic electronic medical records documentation
Candidates able to cover the Webster and/or Nicholas County areas are strongly preferred
