Contract | Full-Time | Days | Monday–Friday with one weekend day rotation every 4 weeks

Key Responsibilities

• Provide skilled physical therapy services to home health patients across an assigned territory including Hartford, West Hartford, Bloomfield, Avon, Simsbury, Granby, Suffield, Windsor, Windsor Locks, East Hartford, Wethersfield, and Rocky Hill
• Complete and maintain OASIS assessments in accordance with home health regulations and documentation standards
• Document patient care accurately and in a timely manner using the designated EMR system
• Meet productivity expectations of approximately 28 points per week
• Oversee and direct the work of Physical Therapist Assistants (PTAs) as applicable
• Participate in a weekend rotation of one day (Saturday or Sunday) every 4 weeks

Required Qualifications

• Active Physical Therapist (PT) license in the state of Connecticut
• Experience with OASIS documentation in a home health setting
• Prior home health experience required
• Proficiency with HCHB (HomeCare HomeBase) EMR
• Ability to travel within the assigned territory (up to approximately 40 miles from the office)

Preferred Experience / Skills

• Experience supervising or directing Physical Therapist Assistants
• Strong time management and self-directed work skills suited for a field-based role
• Familiarity with home health productivity and point-based systems
