    Contract | Days | Full-Time | 40 hours per week | 6-month contract

    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
    • Conduct 60-minute evaluations and 30-minute follow-up sessions; progress patients toward discharge within an established length of stay
    • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
    • Delegate and direct all discipline-specific therapy services for assigned patients; ensure care is provided under signed, valid physician orders and within insurance approvals
    • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
    • Adhere to discipline-specific State Practice Act and all regulatory agency requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
    Required Qualifications
    • Active Physical Therapist license in good standing in the state of Colorado
    • Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans
    • Proficiency in clinical documentation including goal writing and compliance with state and federal guidelines
    • Commitment to patient and employee safety standards including safe patient handling and infection control protocols
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting
    • Experience supervising or mentoring physical therapy students
    • Demonstrated commitment to ongoing professional development through continuing education and professional organization involvement
