    Key Responsibilities
    • Provide physical therapy services in a hospital-based outpatient setting as the primary area of practice
    • Perform occasional inpatient acute and swing bed physical therapy coverage as needed
    • Evaluate patients, develop individualized treatment plans, and monitor progress toward functional goals
    • Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
    • Collaborate with interdisciplinary care teams to coordinate patient care across service lines
    Required Qualifications
    • Minimum 2 years of current Physical Therapy experience required — no new graduates
    • Outpatient Physical Therapy experience required
    • Active New Hampshire Physical Therapist license — must be current and in hand at time of submission
    • Active American Heart Association (AHA) BLS certification — must be current and in hand at time of submission
    • Completed Physical Therapy skills checklist required
    • One supervisory signed reference from the past year of employment required
    Preferred Experience / Skills
    • Inpatient acute care Physical Therapy experience highly preferred
    • Experience working in a Critical Access Hospital or rural healthcare environment
    • Familiarity with swing bed programs and multi-setting PT coverage
