Key Responsibilities
- Provide physical therapy evaluation and treatment services to residents in a skilled nursing setting
- Develop and implement individualized care plans in accordance with facility and regulatory standards
- Collaborate with interdisciplinary team members to support patient progress and outcomes
- Maintain accurate and timely clinical documentation
- Must carry and present a valid state or federal issued photo identification upon entry to the facility and wear a photo badge at all times while on site
Required Qualifications
- Active Physical Therapist (PT) license in the state of New Hampshire
- Graduate of an accredited Physical Therapy program
- Ability to commit to a 14-week contract assignment
- Expected 40 hours per week, Monday through Friday
Preferred Experience / Skills
- Prior experience working in a skilled nursing or long-term care environment
- Familiarity with Medicare documentation and compliance requirements
- Strong communication and team collaboration skills
