Key Responsibilities
- Provide direct physical therapy services to residents in a skilled nursing facility setting
- Evaluate, develop, and implement individualized treatment plans to support patient recovery and functional goals
- Monitor and document patient progress in accordance with facility and regulatory standards
- Maintain valid state-issued Physical Therapist license throughout the duration of the assignment
- Adhere to facility policies, including wearing a photo ID badge at all times while on site
Required Qualifications
- Active Physical Therapist (PT) license in the state of New Hampshire
- Valid state or federal issued photo identification required for facility check-in
- Ability to commit to a 14-week contract assignment
Preferred Experience / Skills
- Prior experience in a skilled nursing facility or long-term care environment
- Familiarity with Medicare documentation and compliance requirements
