Key Responsibilities
- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward therapy goals
- Collaborate with interdisciplinary care teams to coordinate patient care
- Educate patients and families on therapeutic exercises and home programs
Required Qualifications
- Active Physical Therapist (PT) license in the state of North Carolina
- Graduate of an accredited Physical Therapy program
- Experience working in a SNF or similar clinical setting
- Current CPR/BLS certification
Preferred Experience / Skills
- Prior experience with geriatric patient populations
- Familiarity with skilled nursing facility documentation and compliance requirements
