Contract | Full-Time | Days | Monday–Friday, 8-hour shifts | Expected 40 hours/week
Key Responsibilities
- Travel to patients' homes in the Brookings area to deliver physical therapy services
- Implement individualized treatment plans under the direction of a supervising Physical Therapist
- Monitor and document patient progress, adjusting interventions as appropriate
- Communicate effectively with the care team regarding patient status and goals
Required Qualifications
- Active PTA license in the state of Oregon (or eligibility to obtain)
- Ability to travel independently to patient homes within the Brookings area
- Valid driver's license and reliable transportation
Preferred Experience / Skills
- Previous in-home or home health therapy experience
- Strong organizational and time-management skills for managing a home-visit caseload
