Contract (13 Weeks) | Full-Time | Days | Monday – Friday | Weekend Rotation Approximately Every 6 Weeks
Key Responsibilities
- Provide skilled physical therapy services to patients in a home health setting within the assigned territory (Fairfield County)
- Complete required clinical documentation using HCHB electronic medical records system
- Perform OASIS assessments accurately and in accordance with regulatory requirements
- Meet productivity expectations of 28–30 points per week
- Oversee and supervise the work of Physical Therapist Assistants (PTAs)
- Participate in weekend rotation (currently approximately every 6 weeks)
Required Qualifications
- Active Physical Therapist (PT) license in the state of Connecticut
- Prior home health experience required
- Proficiency with HCHB (Homecare Homebase) EMR system
- Experience completing OASIS assessments
- Ability to travel within Fairfield County service area
Preferred Experience / Skills
- Experience supervising or directing Physical Therapist Assistants (PTAs)
- Familiarity with home health productivity standards and point-based documentation systems
