Key Responsibilities

• Evaluate and treat residents in a skilled nursing facility setting
• Develop and implement individualized physical therapy treatment plans
• Monitor and document patient progress toward established goals
• Collaborate with interdisciplinary care team to ensure quality resident outcomes
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of Alabama
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification

Preferred Experience / Skills

• Prior experience working in a skilled nursing facility or long-term care environment
• Experience with geriatric patient populations
• Familiarity with MDS/PPS documentation requirements
