ResponsibilitiesEvaluate patients and develop individualized treatment plans
Implement therapeutic interventions to improve mobility, function, and quality of life
Monitor patient progress and adjust treatment plans as needed
Collaborate with interdisciplinary team members to achieve optimal outcomes
Maintain accurate and timely documentation
Educate patients, families, and caregivers regarding treatment plans and goals
Ensure compliance with all applicable regulations and professional standards
Licensed Physical Therapist (or eligible for state licensure)
Graduate of an accredited Physical Therapy program
Strong communication and interpersonal skills
Ability to work effectively in a team-oriented environment
Commitment to providing exceptional patient care
Experience in skilled nursing, rehabilitation, or long-term care settings
Experience developing individualized treatment plans
