    Key Responsibilities
    • Evaluate assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
    • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities.
    • Complete thorough medical record documentation in a clear, concise, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
    • Ensure all care is provided under signed, valid physician orders and within insurance approvals as required for patient care.
    • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
    • Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing.
    Required Qualifications
    • Active and unrestricted Physical Therapist license in the state of New Hampshire.
    • Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans with appropriate discharge goals.
    • Proficiency in medical record documentation in compliance with state, federal, and payer guidelines.
    • Commitment to patient and employee safety standards, including safe patient handling and infection control practices.
    • Adherence to discipline-specific professional code of conduct and ethical standards.
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting.
    • Experience supervising or mentoring physical therapy students.
    • Active involvement in continuing education and professional development activities.
