Key Responsibilities- Provide physical therapy evaluations and treatments to home health patients
- Develop and implement individualized care plans based on patient needs and goals
- Travel to patient homes to deliver therapy services (approximately 25% travel required)
- Monitor and document patient progress, adjusting treatment plans as necessary
- Collaborate with interdisciplinary care teams to ensure coordinated patient care
Required Qualifications- Active California Physical Therapist license required
- Current Basic Life Support (BLS) certification required
- Ability to travel approximately 25% as part of regular job duties
- Graduate of an accredited Physical Therapy program
