Key Responsibilities
- Provide skilled physical therapy services to patients in a home health setting
- Evaluate, develop, and implement individualized treatment plans to support patient recovery and functional goals
- Travel to patient homes within the designated service area (approximately 25% travel required)
- Document patient progress and maintain accurate clinical records in accordance with regulatory and organizational standards
- Collaborate with interdisciplinary care teams to coordinate patient care
Required Qualifications
- Active California Physical Therapist (PT) license required
- Basic Life Support (BLS) certification required
- Valid driver's license and reliable transportation for patient home visits
Preferred Experience / Skills
- Prior experience in a home health or community-based therapy setting
- Strong time management and independent clinical decision-making skills
