Key Responsibilities
- Provide physical therapy services across both skilled nursing facility (SNF) and home health settings on an approximately 50/50 basis
- Supervise and support two Physical Therapist Assistants (PTAs)
- Utilize SNF-based Net Health software and home health HCHB software for documentation and patient management
- Meet productivity expectations of 80% in the SNF setting and a points-based system of approximately 80 minutes per patient in the home health setting
- Assist with coverage in additional service areas as needed based on PTO and census demands
Required Qualifications
- Active Physical Therapist (PT) license in the state of Nebraska or ability to obtain prior to start
- Ability to work across both SNF and home health care settings
- Proficiency or willingness to learn Net Health (SNF) and HCHB (Home Health) documentation platforms
- Must live within 80 miles of the facility to be eligible for local placement consideration
Preferred Experience / Skills
- Prior experience in a skilled nursing facility or home health setting
- Experience supervising PTAs or other therapy support staff
- Familiarity with productivity and points-based documentation systems
