    Key Responsibilities
    • Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in alignment with patient goals and physician direction
    • Manage 60-minute evaluations and 30-minute follow-up appointments within an outpatient setting
    • Complete accurate and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
    • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support return to community activities
    • Serve as an educator for patients, families, students, and coworkers; participate in student programs with appropriate supervision and evaluation
    • Adhere to the applicable State Practice Act, maintain professional licensure in good standing, and uphold discipline-specific professional codes of conduct
    Required Qualifications
    • Active Physical Therapist license in good standing in the state of Colorado
    • Completion of all state-mandated continuing education requirements
    • Ability to perform comprehensive evaluations and develop evidence-based treatment plans
    • Proficiency in medical record documentation in compliance with state, federal, and intermediary guidelines
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting
    • Experience supervising or mentoring PT students
    • Familiarity with performance improvement methodologies and quality initiatives
