    Key Responsibilities
    • Evaluate, treat, and develop patient care plans in accordance with Physical Therapy standards of practice for adult and geriatric populations
    • Manage a caseload with a variety of orthopedic, neurological, and functional mobility diagnoses
    • Document patient care using point-of-service laptop and NetSmart EMR to meet regulatory and reimbursement requirements
    • Meet productivity expectations of an average of 4.5 visits per day
    • Participate in a rotating Saturday schedule (approximately once every 6–8 weeks); a weekday off is provided in exchange — no holiday rotation required
    • Occasional travel to other branch locations as needed; mileage reimbursed at the government-approved rate
    Required Qualifications
    • Bachelor's degree required; Master's degree in Physical Therapy preferred per practice standards
    • Active Physical Therapist licensure in the State of Wisconsin required
    • AHA BLS certification required
    • NPI number required — candidates without one must initiate the application process prior to start
    • Home health experience required
    • Must reside outside a 50-mile radius of the facility to be eligible
    Preferred Experience / Skills
    • Experience working with adult and geriatric populations across orthopedic, neurological, and functional mobility diagnoses
    • Familiarity with NetSmart EMR or similar point-of-service documentation platforms
    • Adaptable, resilient, and comfortable managing change in a dynamic home health environment
    • Strong interpersonal and communication skills suited for community-based care settings
