    Key Responsibilities
    • Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare and state standards
    • Develop, implement, and update individualized plans of care aligned with physician orders and patient needs
    • Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency-defined timeframes
    • Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts
    • Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety
    • Apply ICD-10 coding accurately for compliant documentation and participate in Quality Assessment and Performance Improvement (QAPI) activities
    Required Qualifications
    • Graduate of an accredited physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation
    • Current Physical Therapist license in the state of New Mexico
    • Current CPR certification
    • Reliable transportation, valid driver's license, and auto insurance required for home visits
    • Commitment to compliance with Medicare/Medicaid Conditions of Participation, state regulations, and agency policies
    • Basic computer proficiency and willingness to learn EMR systems and OASIS documentation
    Preferred Experience / Skills
    • Minimum two years of clinical experience as a Physical Therapist
    • Prior home health experience is a plus, though not required
    • Strong communication and interpersonal skills with sensitivity to diverse patient backgrounds
