Shifts: Mon-Fri 8-5. No on call or weekends

Seven Healthcare are seeking a skilled and dedicated Physical Therapist for a contract assignment in Clarksburg, WV. Specializing in providing exceptional physical therapy care, this role offers an opportunity to work in a supportive environment focused on patient recovery and independence. This is an onsite position covering several counties, and preferred candidates will have home health experience with OASIS and HCHB.

Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for diagnosis, history, precautions, and contraindications
• Establish measurable goals and treatment plans for maximum patient independence/function
• Regularly reassess treatment plan effectiveness and patient progress
• Communicate patient needs and progress to treatment team and family as applicable
• Consult with medical providers regarding treatment and discharge planning
• Recommend and assist with adaptive equipment to enhance rehab programs
• Maintain documentation as required by local, state, and federal regulations
• Supervise Physical Therapist Assistants and Aides in patient care activities
• Participate in staff development programs and obtain continuing education

Requirements of the Physical Therapist

• WV PT License
• Home Health experience with OASIS and HCHB preferred
