Key Responsibilities
Evaluate patients and develop individualized physical therapy treatment plans
Deliver skilled physical therapy interventions in accordance with physician orders and facility protocols
Monitor and document patient progress, adjusting treatment plans as clinically appropriate
Collaborate with interdisciplinary care team members to support patient goals and outcomes
Maintain accurate and timely clinical documentation in compliance with regulatory standards
Required Qualifications
Active Physical Therapist (PT) license in the state of Nebraska or eligibility for licensure
Graduate of an accredited Physical Therapy program
Must reside at least 80 miles from the facility to be eligible for standard contract rates
No on-call or call-back availability required
Preferred Experience / Skills
Prior experience in a skilled nursing or long-term care setting
Familiarity with geriatric patient populations and restorative therapy approaches
Strong communication and interdisciplinary teamwork skills
