Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals.
• Complete comprehensive evaluations using appropriate evaluation and assessment tools and procedures.
• Review patient records for current diagnosis, past medical history, precautions, and contraindications.
• Establish measurable goals and develop treatment plans to enhance patient independence or function.
• Regularly reassess the effectiveness of treatment plans and progress towards goals.
• Communicate patient needs and progress to the treatment team and relevant parties.
• Consult with medical providers regarding patient status and discharge planning.
• Recommend and assist in securing special adaptive equipment for patients.
• Maintain and submit documentation according to required standards and regulations.
• Supervise Physical Therapist Assistants and Aides in patient care activities.
• Participate in internal staff development programs and maintain clinical practice skills.

Requirements of the Physical Therapist

• Valid Physical Therapy license
• Experience in Home Health is a significant plus
• Proficiency in Home Care Home Base documentation is a must
