Key Responsibilities of the Physical Therapist- Perform physical therapy evaluations and treatments based on physician referrals.
- Complete comprehensive evaluations using appropriate tools and procedures.
- Review patient records and establish measurable goals and treatment plans.
- Regularly reassess treatment effectiveness and patient progress.
- Communicate patient needs and progress to the treatment team and relevant parties.
- Consult with medical providers regarding patient evaluations and progress.
- Recommend and assist with adaptive equipment for patient improvements.
- Maintain and submit required documentation according to regulations.
- Supervise Physical Therapist Assistants and Aides in patient care activities.
- Participate in staff development programs and maintain clinical practice skills.
Requirements of the Physical Therapist- Must have licensure in Physical Therapy
- Experience with Oasis and Home Health is a huge plus
- Proficiency in HCHB software is required
