Collaborate with patients, families, physicians, and other allied health professionals. Identify patient needs and coordinate services to meet those needs. Utilize thorough and timely electronic documentation. Engage in interdisciplinary communication in accordance with agency policies and standards of practice. Weekend rotations are required every 6 weeks. Ability to travel to assigned cases is necessary. Experience in hospice, private care, or in certain case management roles does not meet the qualifications for certified home health care management experience. Only roles specifically within a certified home health agency will be considered relevant for the experience requirement.
