Description:
Seeking a full-time physical therapist in a hospital setting.
This position focuses on acute care and inpatient rehab as well as outpatient rehab.
Responsibilities include assessing, planning, organizing, and participating in rehabilitative programs.
The goal is to improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.
The therapist will gain confidence and cooperation from patients, their families/support groups, and other healthcare providers through competent patient assessment, attentive monitoring, and effective communication.
Adheres to all local, state, and federal regulations, codes, policies, and procedures to ensure privacy and safety while delivering optimal patient care.
Provides clinical supervision of assistants and aides and serves as a clinical instructor for assigned students.
Plans, prepares, and implements individually designed physical treatment programs to maintain, improve, or restore physical functioning.
The role includes completing and documenting patient evaluations and tests according to established standards and regulatory requirements.
Develops and updates the patient's Plan of Care based on the data, identified problems, and diagnoses from the exam.
Assesses and provides/progresses treatment for patients per the plan of care according to established standards.
Completes all documentation related to charge capture according to established standards.
Coordinates patient care effectively and efficiently with other care providers and support team members.
Maintains a clean, neat, and safe working environment, including cleaning and disinfecting equipment after each use.
Participates in quality improvement processes and interdisciplinary projects.
Maintains strict confidentiality of all patient protected health information and protects all information from accidental, intentional, or inappropriate disclosure.
Prepares, administers, and documents services according to state and federal regulations.
Retains certification and completes mandated continuing education requirements to meet facility and regulatory agency accreditation and licensure requirements.
Education: Graduate of an accredited physical therapy graduate program.
Skills: Experience with various rehabilitation techniques and treatments.
Demonstrated ability to work collaboratively in a team-based environment.
Effective communication skills with patients, families, and healthcare providers.
