Key Responsibilities of the Physical Therapist - Long Term Care - Travel- Assess patients' physical conditions to develop an appropriate treatment plan
- Implement treatment plans that improve mobility and alleviate pain
- Monitor patient progress and modify plans as necessary
- Document patients' conditions and progress accurately
- Educate patients and their families on rehabilitation and exercises
