We are seeking an experienced and dedicated Travel Physical Therapist (PT) for a 13-week contract assignment in an outpatient setting. The PT will provide high-quality physical therapy services to help patients improve mobility, strength, balance, endurance, and overall functional independence.
Approximately 20% of visits may take place in Assisted Living Facilities (ALFs), so candidates must be comfortable providing therapy across multiple settings. The ideal candidate will have recent outpatient and homecare/home-based experience, along with previous travel therapy experience.
Assignment Details- Schedule:Monday–Friday, Days
- Guaranteed Hours:40 hours/week

- Facility Work Week: Sunday–Saturday
- Missed Visits: May be made up on weekends
- Daily Stand-Up Meeting:8:30 AM Monday–Friday
- Travel Radius:50 miles from the facility
- Expected Weekly Mileage: Approximately 50–75 miles, but may vary
- Mileage: Included in the bill rate

Responsibilities- Perform comprehensive physical therapy evaluations and develop individualized treatment plans
- Provide therapeutic exercises, gait training, balance training, manual therapy, neuromuscular re-education, transfer training, and functional mobility interventions
- Provide therapy services in the outpatient setting and Assisted Living Facilities as assigned
- Utilize homecare/home-based experience to effectively evaluate and treat patients in different environments
- Monitor patient progress and modify treatment plans based on clinical needs and outcomes
- Educate patients and caregivers on home exercise programs, mobility techniques, fall prevention, and safety strategies
- Maintain accurate and timely documentation using WellSky
- Participate in required 8:30 AM daily stand-up meetings
- Collaborate with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary healthcare team
- Manage an assigned caseload while maintaining quality patient care and productivity expectations
- Complete required visits within the designated service area
- Maintain a professional appearance and comply with facility dress and safety standards
Requirements- Required: Graduate of an accredited Physical Therapy program
- Required: Active Physical Therapist (PT) license in the state of assignment
- Required:Minimum 2 years of recent licensed PT experience
- Required:At least 1 recently completed travel assignment
- Required: Previous homecare/home-based therapy experience
- No New Graduates
- Required:BLS certification
- Required:First Aid certification
- Strong clinical reasoning, communication, documentation, and organizational skills
- Ability to travel between patient locations within the assigned service area
- Ability to work independently and adapt quickly to different clinical environments
Additional Assignment Requirements- Clean, professional scrubs with closed-toe shoes are required
- COVID-19 vaccination and boosters are required
- Medical or religious exemptions may be considered on a case-by-case basis by the facility
- Travelers approved for an exemption will be required to undergo weekly testing, with testing handled by the facility
- Travelers should research and confirm suitable housing options before submission, as the assignment requires travel within the designated service area
