Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based on physician orders/referrals.
• Complete comprehensive evaluations using appropriate tools and procedures.
• Review patient records for diagnosis, past history, precautions, and contraindications.
• Establish measurable goals and develop treatment plans for patient independence or function.
• Regularly re-assess treatment plan effectiveness and patient progress, modifying as necessary.
• Communicate patient needs and progress to the treatment team and relevant parties.
• Consult with medical providers about patient treatment and discharge plans.
• Recommend and assist in obtaining adaptive equipment to enhance rehabilitation.
• Maintain and submit documentation per required standards and regulations.
• Adhere to the Company's Code of Business Conduct and professional practice standards.
• Supervise Physical Therapist Assistants and Aides during patient care activities.
• Participate in staff development programs and obtain necessary continuing education.

Requirements of the Physical Therapist

• Licensed NC PT or compact
• Experience in home health care
• Familiarity with HCHB and OASIS
