Key Responsibilities of the Physical Therapist- Perform physical therapy evaluation and treatment based on physician orders/referrals
- Complete comprehensive evaluations using proper assessment tools
- Review patient records for diagnosis and medical history
- Establish measurable goals and develop treatment plans
- Re-assess treatment effectiveness and modify as needed
- Communicate patient progress to the treatment team and relevant parties
- Consult with medical providers about patient progress and discharge plans
- Recommend and obtain adaptive equipment to enhance rehabilitation
- Maintain evaluation, treatment, and billing documentation as required
- Supervise Physical Therapist Assistants and Aides
- Attend and conduct staff development programs
- Maintain professional practice standards and comply with regulations
Requirements of the Physical Therapist- Ga License (Compact license accepted)
- Proficiency in HCHB
- Previous home health experience
