Seven Healthcare are seeking a dedicated Physical Therapist for a contract assignment in Cedartown, GA. This role specializes in delivering comprehensive physical therapy services within Bartow and Paulding Counties. Key Responsibilities of the Physical Therapist
  • Performs physical therapy evaluation and treatment of patients based upon physician orders/referrals.
  • Completes a comprehensive evaluation using appropriate evaluation and assessment tools and procedures.
  • Reviews patient records for current diagnosis, past medical history, precautions and contraindications.
  • Establishes measurable goals and develops treatment plans to maximize patient independence/function.
  • Regularly re-assesses treatment effectiveness and modifies goals or treatment as needed.
  • Communicates patient progress to treatment team, physician, family members as applicable.
  • Consults with medical providers on patient evaluation, treatment, and discharge.
  • Recommends and secures special adaptive equipment to improve patient function.
  • Maintains and submits documentation as required by regulations and company policy.
  • Adheres to professional practice standards and applicable laws.
  • Supervises Physical Therapist Assistants and Aides in patient care activities.
  • Participates in staff development programs and maintains clinical practice skills.

Requirements of the Physical Therapist
  • Valid PT License
  • Experience with HCHB and OASIS
