Key Responsibilities of the Physical Therapist- Perform physical therapy evaluations and treatments based on physician orders/referrals
- Conduct comprehensive evaluations using appropriate tools and procedures
- Review patient records for diagnoses, medical history, precautions, and contraindications
- Establish measurable goals and develop treatment plans for patient independence and function
- Re-assess treatment efficacy and patient progress, modifying as needed
- Communicate patient needs and progress to treatment team, physicians, and families
- Consult with medical providers regarding evaluations, treatments, and discharges
- Recommend and secure adaptive equipment to enhance rehabilitation programs
- Maintain and submit documentation as required by regulations and company policy
- Supervise Physical Therapist Assistants and Aides
- Participate in staff development and continue education to enhance skills
Requirements of the Physical Therapist- Valid PT license
- Experience with HCHB and OASIS
- Productivity requirement of 30 points
