Description:
The Physical Therapist is responsible for evaluation and rehabilitation intervention in the home setting.
This includes management and supervision of patient care and patient and family education.
The therapist evaluates and establishes a plan of care to meet patients' needs in the home environment.
Care is coordinated with other disciplines, and orders are sought for appropriate additional services.
Direct patient care is provided based on the established care plan and physician orders.
Knowledge of physical therapy evidence-based practice guidelines and regulations is applied in evaluations, re-evaluations, and treatment techniques.
Treatment is modified as appropriate, including frequency and duration of care.
The therapist masters scoring of required assessments and collaborates with a quality team to make necessary adjustments.
A basic understanding of the insurance requirements in home health is maintained.
The therapist is willing to perform interventions allowed under the physical therapy practice act, which may include wound care.
Participation in multidisciplinary conferences is expected to establish appropriate patient-relevant care plans.
Effective communication with physicians and other staff members is vital.
Point of care documentation is practiced in all direct patient care settings.
All skilled services provided to patients, adverse events, and relevant communications are documented in the medical record.
Participation in quality assurance performance improvement initiatives is necessary to assist the agency in meeting its goals.
Productivity standards in line with the agency’s standards must be met.
Accurate time logs and mileage must be maintained.
Polices for commuting and logging mileage are to be followed.
Daily communication with the scheduler is required.
Maximizing continuity of care and efficient travel routes is essential when preparing schedules.
Services provided by the Physical Therapist Assistant are overseen, and supervisory visits are performed within the required time frame.
Supervision of the Certified Nursing Assistant is required when the Registered Nurse is no longer involved in the care plan.
The therapist participates in relevant care conferences.
Appropriate planning for admissions, time points, and discharges throughout the patient’s episode of care is necessary.
Requirements of physical therapy licensure, including continuing education standards, must be maintained.
The therapist practices with expected standards of ethical behavior as defined by the physical therapy licensure board and agency policy.
Participation in agency-provided educational opportunities is scheduled.
Other duties may be assigned by the supervisor.
Two years of previous physical therapy experience is preferred.
