Description

Job Title: Home Health Physical Therapist Profession: Physical Therapist Specialty: Home Health Duration: 13 Weeks Shift: Monday-Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of one year in physical therapy preferred License: Currently licensed as a Physical Therapist in the state and in good standing Certifications: Current automobile insurance and valid driver’s license Must-Have: Knowledge of principles, concepts, and methodology of physical therapy. Demonstrates problem-solving and decision-making abilities. Ability to manage patient care with specific knowledge and experience in symptom management and family intervention. Excellent communication skills. Ability to develop and maintain rapport with patients, families, and staff. Description: The Physical Therapist reports to the Director of Professional Services. This role is responsible for providing medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, company policies, and professional standards of practice. Participates in the development of patient physical therapy plans of care. Makes visits to patients for assessments and evaluations and communicates with the physician before the start of care as needed. Implements the plan of care. Documents the care and submits paperwork accurately and timely to update and maintain patient medical records. Provides education and instruction to team members, patients, and their families. May participate in agency quality assessment performance improvement programs. Accepts other assignments deemed appropriate. Ensures the growth and profitability of the company through responsible use of company resources and educating the community about services. Applies policies and procedures of the agency and the rules and regulations of federal and state regulatory agencies in providing care. Provides skilled restorative treatments for pain reduction and develops, restores, and maintains functions. Treatments include moist heat, whirlpool uses, paraffin baths, gait training, ultrasound, electrical muscle stimulation, and range of motion testing. Instructs, establishes, and supervises exercise programs, prosthetic training, balance and coordination training, and cardiovascular rehabilitation. Instructs in home safety. Conducts initial and ongoing comprehensive assessments of patient needs. Evaluates outcomes of care. Directs and supervises Physical Therapist Assistants and Home Health Aides in care provided to patients. Provides supervisory visits as necessary. Willingly accepts direction from the Director of Professional Services and other supervisors.
