Job Opportunity – Physical Therapist, Home Health – Medford, OR 97504
Start Date: September 21, 2026
Location: Medford, Oregon and surrounding areas
Schedule: Day shift, 8-hour shifts (08:00 - 16:30), Monday through Friday. Up to 6 patient visits per day with a standard productivity of 30 points. No weekend coverage required. No on-call requirements.

Requirements:- Active Oregon Physical Therapy license or compact license (must be in hand at time of submission)
- Minimum 1 year of skilled home health experience required
- BCLS (Basic Cardiac Life Support) certification required
- Oasis documentation experience required
- COVID-19 primary vaccination series plus one booster required
- Reliable personal transportation required for patient visits (occasional travel up to 50+ mile radius)
- Two professional references covering 1 year within the last 3 years
