Utilize thorough and timely electronic documentation. Engage in interdisciplinary communications in accordance with agency policies and standards of practice. Weekend rotations may be required, occurring once every 6 weeks. Ability to travel to assigned cases is necessary. 1 Year of Certified Home Care experience is required. 1 Year of Case Management experience is also required. Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement.
