Seven Healthcare are seeking a passionate and skilled Physical Therapist to join a dynamic home health team in Brevard County, FL. This is a full-time field-based role, where you will be providing high-level care such as SOC, evaluations, reassessments, and discharges to patients across multiple agency partners. The work environment is flexible and autonomous with strong administrative and scheduling support.

Key Responsibilities of the PT, Home Health
Conducting SOC assessments, evaluations, reassessments, and discharges
Coordinating schedules and exact visit times with patients
Completing documentation within partner agency EMRs
Providing orthopedic visit coverage on Saturdays
Collaborating with PTAs for routine visits
Adapting to work across multiple home health agency environments
Requirements of the PT, Home Health
Valid Physical Therapy license in the state of Florida
Experience in home health care preferred
