Travel Physical Therapist (PT) - $2,670 per week in Windsor, VT

Job Title: Physical Therapist Profession: Physical Therapist Specialty: Inpatient Rehabilitation Duration: 13 Weeks Shift: Tuesday - Saturday or Sunday - Thursday Hours per Shift: 8 Hours Experience: Minimum of 2 years of current inpatient experience License: Active Vermont Physical Therapy license Certifications: Active American Heart Association BLS Must-Have: - 2+ years current physical therapist inpatient experience - At least 6 months previous travel experience - Experience with Cerner preferred - Traveler must have their own vehicle Description: Responsible for providing physical therapy treatments to patients based on an evaluation of their needs and status. Maintains appropriate records and assists with in-services and community health programs. Requests for time off during the assignment must be noted and verified at the time of submission. No more than 5 days of requests for time off will be accepted. All scheduling requests must be made at the time of submission. Profile requirements include a cover page, work history, and skills checklist. A supervisory signed reference from within the past two years of employment is required. Compliance with attendance policies is essential. Each call-off must be communicated to the unit supervisor at least 1.5 hours in advance. Failure to adhere to attendance policies may result in contract termination.
